    1. Will my SSA Family Health and Safety passport replace my legal passport? - Top

    No, the SSA Family Health and Safety passport is an important tool for spouses to keep track of theirs and their family member’s vaccination records, to keep record of health and safety training, medical information about the employee’s spouse and children, contacts, and important information about the company and what to do in an emergency. There is even a space for keeping track of pet vaccinations and information about moving with pets.

    This document can not replace your legal documents such as: passport and international vaccination cards.

    The spouse of any Schlumberger employee. - Updated: August 22, 2007

    3. Will I have one passport for me and one for each of my children? - Top

    No, the passports will be distributed one per family. There is enough space to include the records of the spouse and four children.

    Additional pages can be added and stored in the plastic cover.

    4. How can I get one? - Top

    Spouses should contact the SSA chapter Coordinator in your location. If there is not a formal SSA Chapter, please contact your Area Coordinator. To know who is your Area Coordinator visit www.ssafara.net/Who_We_Are

    5. Do I have to take the passport to my family’s doctor to include all the vaccinations and medical information? - Top

    No, you can include this information in the passport yourself. Simply write down the details. - Updated: August 22, 2007

    6. What is the procedure to record all the Health and Safety training? - Top

    If you take any training classes or orientation sessions with Schlumberger, take the passport with you to the training.

    Once you have taken the training you can ask the instructor to sign and date the passport.

    You can also enter the information yourself.

    7. Is Schlumberger QHSE required to provide training for Spouses? - Top

    No, Schlumberger QHSE groups are not required to provide any training to spouses. However, Schluberger has some of the the best Health and Safety practices in the industry, so extending available training to families of employees is a logical progression and will also help to reinforce health and safety best practices.

    Training availablility will vary by location, resources, and needs.

    8. What can I do to receive Health and Safety training for spouses? - Top

    Contact your local coordinator, who will coordinate with the local QHSE Manager to decide on the best training needed. They will decide on the date and time, you just have to show interest in them by participating. - Updated: August 22, 2007

    9. All the passports my chapter received were distributed and we now need more, how can I order them? - Top

    Local SSA Coordinators should work with a local employee to place passport orders, which come in packages of 10.

    Any Schlumberger employee can order the passports from InTouch

    10. Are there some specific recommendations when placing an additional order? - Top

    Some recommendations when ordering the passports are:

    a.- The ordering must be done by an employee, not a spouse, because the passports will be shipped to the postal address listed on the employee's LDAP record.

    b.- The InTouch order process allows you to order up to 50 passports. To order more than 50 there are instructions on the InTouch page for submitting a helpdesk ticket.

    c.- The passports come in group of ten. According to the instructions on the InTouch page if you write 1 you will ordering 10, so be careful when ordering them.
